INSPECTION_COMMENT: An incident inspection is being conducted in response to a report of a release of an estimated 50 gallons of brine at the Rowan H. Heyn 3/UW-112 storage well site (API 37-051-00097).  Release occurred on 04/09/21 and was reported to the emergency spill hotline and SWRO (IR # 3182967).  Written notification was submitted on 04/20/21 to DOGM Daniel Counahan.   Met Vanessa Fowler, Eastern Gas Transmission and Storage, Inc (EGTS) Environmental Lead, and Tracy Gardon, EGTS Lead Operator, at the North Summit Compressor Station.  Moved to site at approx. 1300.  Mr. Gardon and Ms. Fowler identified the area affected by the release near the well head.  Mr. Gardon stated that scheduled maintenance was being conducted at this well at time of the incident.  Valves were open in anticipation of pipe welding that was scheduled but the piping delivery was delayed.  An inadvertent pressurization from the side lines to the lead lines resulted in the release at the open valve was confirmed by Mr. Gardon.    Excavation of affected area was still open at time of inspection.  Area was being left open until analysis of samples collected by Groundwater and Environmental Services (GES) are received and confirm area has been remediated.  The Department requests a copy of the sample analysis and final report when completed.  Conductance readings were taken by WQS during inspection.  Background readings outside the affected area were at 130 uS.  Two readings were taken within excavated area.  The first reading of 240 uS was taken in the shallow cut soil(Photo 2).  The second reading of 2020 uS was taken in the soil/standing water in the deeper cut near exposed well casing (Photo 3).  This far exceeds background readings taken outside the release area.   Discussed these findings with Jen McCloskey.  Ms. McCloskey stated that GES would be sent out to conduct additional testing and would continue excavation if conductance and/or sample analysis showed elevated conductance and/or Chlorides.  Well pad and vegetated embankment inspected.  No evidence observed that the release has migrated outside the identified affected area.  Vegetation did not appear stressed, no seepage observed.  No erosion or sedimentation issues were observed on site (Photo 1).  Well tag was in place at well head (Photo 9).  Brine tank appeared structurally stable and was within earthen berm.  Earthen berm embankments were vegetated and appeared stable.  No staining or standing water observed within the earthen berm area (Photos 4&5).  The Department requests that the operator provide any photographs of the incident as it occurred and before remediation began. The Department also requests the waste manifest once it is available.  Based on this inspection, violations are being issued for the release.  Pending the operators response or remediation efforts, more violations may be issued.

INSPECTION_COMMENT: Incident inspection conducted in response to a reported release on site.  Release was reported to PADEP SWRO by Chad Eisenman, Diversified Production LLC., at 1747 on 05/03/21 (CTS ID# 356447).  The time of the release was reported as 1600 on 05/03/21.  The 2 hour report time for a spill was met by Operator.    Operator reported approx. 14 gals of production fluid leaked at the load valve on poly-tank.  Mr. Chad Eisenman was contacted by phone at approx. 0930 on 05/04/21 to gather more information about the spill.  Mr. Eisenman believed the leak at load valve was caused by corroded valve.  The Operator ceased leak by replacing the corroded valve with a plug (Photo 18).  Release was reported as contained to site.  The Operator's initial response to the spill was to remove as much fluid from the brine pad as possible and empty the remaining fluid from brine tank.  Upon inspection, it was observed that liquid was removed from both facilities.  It was also observed that an earthen berm was in place around brine tank and spill did not appear to leave the pad area.  Some standing water was observed along access road (Photo 21) and on pad south of brine tank (Photo 20).    Well tag and posting were in place at the proper locations (Photos 1 & 2).  Out-slopes of the well pad were inspected.  Out-slopes of the pad showed excellent vegetative cover (> 70% uniform).  No evidence of runoff or stressed vegetation was observed around the perimeter of well pad.  WQS took conductance readings along the entire perimeter of well pad (Photos 4-9).  Background conductance readings ranged from 100 uS to 420 uS.  WQS took conductance readings at areas of standing water along access road and on well pad south of brine tank.  These locations showed elevated conductance readings when compared to background of 900 uS (Photos 11 & 19) at time of inspection.  Additional conductance readings were taken by WQS within the earthen berm that collected the spilled brine (Photos 12-18).  The conductance readings were above background ranging from 760 uS to 2380 uS.  Fluid from the release was noted just below the gravel within the earthen berm around tank (Photo 12/13/15/16/17/18).    Contacted Mr. Eisenman and informed him of the findings of the inspection.  Recommended that standing water along access road and south of brine tank be removed from site and properly disposed.  Operator informed WQS that the Company would begin remediation of the spill area on Thursday 05/06/2021.  Plan for remediation was to remove brine tank which, according to the Operator, is no longer needed at this site.  Once brine tank is removed, the soils within the earthen berm will be excavated until clean soils are uncovered.  The Berm material will then be utilized to backfill the area.  Note:  several conductance readings were taken along the earthen berm material by WQS.  Readings were within background levels of this site.  Followup inspection will be conducted to inspect the effectiveness of the remediation effort in the next 30 days.    Based on this inspection, violations are being issued for this spill.  Pending the operators response or remediation efforts, more violations may be issued.  Note:  The Department requests copies of the manifests for all production fluid and contaminated soils removed within 7 business days of final remediation.  The Department also requests a written summary from the operator providing information and/or supporting documentation on how the quantity released (14 gal) was determined.
